New Patient Registration

Our registration process has changed slightly during the COVID-19 period (March 2020 onwards).  We CANNOT take on new patients who already have a GP in the local area.  Patients should stay with their current GP until the COVID pandemic is over.  Only people who DO NOT have a GP locally can register at this time. 

If you DO NOT have a GP and wish to register with Oakleaf Medical please call to the Practice and ask for a New Patient Pack. The forms in the pack are for information and should be read prior to submitting your registration form. All partners are involved in the decision of whether or not a patient is accepted onto the practice list. Once accepted you will be offered a Telephone Consultation with the Practice Nurse.
